PRE-OPENING INSPECTION: FACILITY IS ALMOST READY TO BE GIVEN THE OK TO OPERATE. THE REFERRAL FORM WAS MISSING SOME SIGNATURES. ONCE THESE SIGNATURES ARE OBTAINED AND THE FORM IS RECEIVED BY THE DEPARTMENT, OFFICIAL PERMISSION TO OPERATE CAN BE GIVEN. FOR THE INSPECTION, FACILITY WAS JSUT PATCHING UP SOME OF THE WALL OPENINGS THAT WAS LEFT FOR THE PLUMBING INSPECTION. THE 3-BAY SINK WAS MISSING THE DRYING RACK WHICH THE MANAGER SAID WILL BE PUT UP NEXT AFTER THE WALL OPENINGS ARE COVERED. OVERALL FACILITY HAS EVERYTHING IN PLACE.
ONCE THE REFERRAL IS RECEIVED BY THE DEPARTMENT, THE PERMISSION TO OPERATE REPORT WILL BE SENT OVER AND THE PAYMENT FOR THE LICENSE WILL BE PROCESSES. LICENSE APPLICATION WAS RECEIVED AT THE TIME OF THE PRE-OPENING INSPECTION. |
